Regimen Management

Regimen Management

The regimen is the basis of treatment monitoring in SureAdhere. It fills the patient's Adherence calendar with the doses they are expected to take, and it drives dose reminders. If a patient hasn't submitted a dose video, they get a reminder one to two hours after the scheduled dose time (see Dose reminders below).

This page covers the regimen builder: how to create a regimen, and how to change one safely once treatment has started.

Required role: creating and editing regimens requires the Regimen Editor role. The Client Staff Admin (CSA) role can add the Regimen Editor role during staff creation or updating.

About the regimen builder

The Regimen tab has two halves:

  • Left side: the regimen name, the disease, and a list of drug schedules.

  • Right side: a Calendar preview that updates as you work, before anything is saved. Each drug gets its own coloured dot on the days it's due, and today is highlighted in green. Hover over a day to see that day's drugs, doses, and pill counts (for example, Bedaquiline - 400mg (2 pills)). Use the ‹ › arrows beside the month to move between months.

A few things the builder does for you:

  • Total dose is calculated automatically from the number of pills × dose per pill, which prevents entry errors.

  • Each schedule is saved separately, so changing one drug doesn't disturb the others.

  • Weeks start on Sunday in the calendar and in every date picker.

  • Schedules are listed newest first. Schedules that have already ended are shown in grey.

Create a regimen

  1. Open the patient's account and go to the Regimen tab.

  2. Optionally, enter a regimen name in the name box at the top left.

  3. Choose the disease from the dropdown next to it.

  4. If you entered a name or changed the disease, click SAVE. (SAVE only lights up when one of those two fields has changed.)

  5. Add one or more drug schedules. See the next section.

Add a drug schedule

  1. Click + NEW SCHEDULE. A blank schedule, tagged New Drug Schedule, appears at the top of the list.

  2. Drugs: click the box that reads You can add multiple drugs on this schedule, type, and choose the drug. To save time, you can add several drugs to one schedule at once when they share the same frequency, time, and duration. For a drug with a different frequency, time, or duration, simply add it on its own NEW SCHEDULE.

  3. For each drug you add, enter:

    • Number of pills

    • Dose per pill (mg/ml)

Total dose (mg/ml) is filled in for you.

  4. Start date: pick the first day of treatment.

  5. Time: this defaults to 6:00 PM. Change it to the time of day the dose should be taken. Reminders are based on this time.

  6. Repeats: this appears once a start date is entered, and defaults to Daily. Choose one:

    • Daily: every day.

    • Weekly on [day]: once a week, on the same weekday as the start date.

    • Weekdays: Monday to Friday.

    • Every Other Day: doses alternate days, starting on the start date.

    • Custom: tick the specific days of the week under Select days.

  7. Number of weeks: enter how long the schedule lasts. Decimals are allowed (for example, 2.5).

    • If weeks aren't the right fit, click ▼ Advanced options to enter a Number of days, or pick an exact End date instead. Click ▲ Hide advanced options to close it again.

    • Weeks, days, and end date stay in sync, so use whichever is easiest. The end date is the last day of treatment. For example, 32 weeks starting 2026-09-23 is 224 days, ending 2027-05-04.

  8. Check the Calendar preview on the right to confirm the patient will be expected to take the drug on the right days. The schedule heading also shows the total number of doses, for example Schedule (224 doses in total).

  9. Click CREATE.

Repeat these steps for each additional schedule.

Required details: all of the details above are needed to create a schedule, and Number of pills and Dose per pill must be greater than 0. If anything is left blank when you click CREATE, it's highlighted so you can fill it in and click CREATE again.

More than one dose a day: each schedule has a single dose time. If a drug is taken more than once a day, add a separate schedule for each time (for example, one at 8:00 AM and one at 8:00 PM).

Edit a regimen

Each saved schedule appears as a summary card, for example:

Bedaquiline 200 mg/ml (2 pills)
Daily at 6:00 PM, from Aug 1, 2025 to Aug 11, 2025 – (11 doses over 1.57 weeks)

To make a change:

  1. Click EDIT on the schedule's card.

  2. Change what you need: Number of pills, Dose per pill (mg/ml), Start date, Time, Repeats, or End date. The drug itself can't be changed on an existing schedule. To switch drugs, end this schedule and add a new one.

  3. Click SAVE, or CANCEL to discard your changes.

Changing the Time also moves the patient's reminders to match the new dose time.

Changing a schedule that has already started: "Schedule Change Required"

If a schedule has already started and you change the time, repeats, days, number of pills, or dose per pill, a Schedule Change Required box appears with the note "SureAdhere will end the current drug schedule and start a new one on [date] based on these changes."

  1. Click the Change date box (When should this change take effect?) and pick the day the new instructions should start. It must fall between the schedule's start and end dates.

  2. Click SAVE.

SureAdhere ends the current schedule on the day before the change date, then starts a new schedule on the change date with your new values, running to the original end date. The patient's history before the change date is kept as it was.

Changing only the start or end date does not need a change date. Schedules that start in the future are simply updated.

Stop a drug

To stop a drug, click EDIT on its schedule, set the End date to the last day the patient should take it, then click SAVE. The drug is no longer expected after that date.

Ending a drug early when a patient has more than one drug schedule on the same day

When a patient has several drug schedules with doses on the same day, SureAdhere only checks for reviewed doses when you end the last of those schedules. If you end the other schedules first, their new end dates save without a warning. Those drugs are removed from the Regimen tab but still show on the Adherence tab for days already reviewed, so the two tabs no longer match.

Before ending any drug schedule early, check the Adherence tab and set the End date to the last reviewed day or later.

Delete a schedule

Use DELETE only for a schedule that was entered by mistake:

  1. Click EDIT on the schedule, then DELETE.

  2. In the Confirm Deletion box, click YES, DELETE THIS SCHEDULE. This can't be undone. Click CANCEL to keep the schedule.

A schedule that already has dose details recorded can't be deleted, because that would remove the patient's adherence history. End the schedule instead (see Stop a drug).

Dose reminders

The regimen drives the patient's dose reminders. If a patient hasn't submitted a video for a dose, SureAdhere sends one reminder one to two hours after the scheduled dose time. For example, for a dose scheduled at 8:00 AM, the reminder arrives between 9:00 and 10:00 AM. SureAdhere checks for overdue doses once an hour, so the exact time within that window varies.

  • If the patient has already submitted their video, no reminder is sent.

  • The reminder reads "Your dose is past due, please take your medications now." in the patient's preferred language.

  • If you change a schedule's Time, the reminders move with it.

How the reminder is delivered: each reminder is sent one way or the other, not both.

  • In-App: if In-App messages and reminders are both enabled, the patient gets the reminder in the SureAdhere app, with a notification on their phone.

  • SMS: if the patient is opted in to SMS instead, they get the reminder by text message.

Reminders are on by default. When a new patient is created, SureAdhere automatically opts them in to reminders. You don't need to do anything to turn them on.

Turn reminders off (or back on) for a patient

  1. Open the patient's account and go to the Profile tab.

  2. Under Settings, find Reminder settings.

  3. Switch the toggle to OFF, or back to ON.

  4. Click SAVE at the bottom of the page.

Turning reminders off stops all reminders for that patient. The patient's regimen and Adherence calendar are not affected; only the reminders stop.
