Create or select a custom form a form export: Information to create a custom form a form export is located here. To select an existing saved custom form export go to:
CommCareHQ -> Data -> Export Data -> Export Forms -> Custom Exports and select “Edit” for the export you want to download with de-identified data. Scroll to the bottom of the page to the Privacy Settings.
